Please follow these steps to input the following information into the contribution template accurately:
A. Employer Name (Legal Trade Name): Please enter your company's legal trade name.
B. Employer Number (Assigned by the Silver Thatch Pension Plan): Include the unique employer number assigned to you by the Silver Thatch Pension Plan.
C. Contribution Period From (Date Must Match Payroll Start Date): Please specify the contribution period's starting date, making sure it aligns with your payroll's commencement date. Please enter in the following format: MM/DD/YYYY.
D. Contribution Period To (Date Must Match Payroll End Date): Indicate the contribution period's end date, corresponding to your payroll's conclusion date. Please enter in the following format: MM/DD/YYYY.
E. Cheque Number/Wire Date (Enter the Cheque Number or Date the Funds Were Transferred): Record either the cheque number or the date when the funds were transferred.
F. Currency (Select the Currency in Which Funds Are to Be Allocated): Choose the appropriate currency for allocating funds.
G. Member Number (Assigned by the Silver Thatch Pension Plan): Input the unique member number assigned to each employee by the VSilver Thatch Pension Plan.
H. Last Name (Member's Legal Last Name as Displayed on Their ID): Enter the employee's legal last name as it appears on their identification.
I. First Name (Member's Legal First Name as Displayed on Their ID): Input the employee's legal first name, as indicated on their identification.
J. Employee Mandatory Contribution (Enter the Amount Deducted from the Member's Salary): Specify the amount deducted from the member's salary as their mandatory contribution.
K. Employer Mandatory Contribution (Amount Must Match the Amount Deducted from the Member's Salary or Be No Less than 5% of the Member's Salary): Provide the employer's mandatory contribution, ensuring it matches the deduction from the member's salary or is at least 5% of the member's salary.
L. Employee Voluntary Contribution (Contributions the Member Has Elected to Pay in Addition to the Mandatory Amount): Record any additional contributions the member has chosen to pay, over and above the mandatory amount.
M. Employer Voluntary Contribution (Contributions the Employer Has Elected to Pay Towards the Mandatory Amount): Document contributions that the employer has decided to contribute toward the mandatory amount.
N. Property Repayment (Additional 1% Deduction Towards Repayment of the Property Withdrawal, Applicable Only to Those Who Have Participated in the Property Withdrawal Under Section 52A): If applicable, indicate the additional 1% deduction for property withdrawal repayment for eligible members under Section 52A.
O. Termination Date (Enter Termination Date if the Member's Employment Has Ended): Include the termination date if the member's employment has ceased.
